Who Should Not Take a GLP-1 Weight Loss Medication? A Safety Guide for Dallas Adults

Medically reviewed by Dr. Diala Alatassi, MD

GLP-1 medications are not appropriate for everyone. Here are the main conditions and situations that may rule them out or call for extra caution, and why a medical review comes first.

Weight-loss medications can be effective for some adults, but they are not for everyone. A careful medical history is the first step, and it is one reason these medications should be prescribed by a licensed provider.

Boxed warning: thyroid C-cell tumors

Wegovy, Zepbound, and Saxenda carry a boxed warning about the risk of thyroid C-cell tumors seen in rodents. They should not be used by people with a personal or family history of medullary thyroid carcinoma or with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). See the labels for Wegovy and Zepbound.

Other situations that call for care

  • Known allergy to the medication or its ingredients.
  • Pregnancy or planning pregnancy. Weight-loss medications are not recommended in pregnancy, and your provider will discuss timing.
  • Breastfeeding. Discuss with your provider.
  • History of pancreatitis or gallbladder disease.
  • Kidney disease, especially if you are dehydrated.
  • Diabetic retinopathy, particularly with rapid blood-sugar changes.
  • Severe digestive conditions, such as gastroparesis.
  • Diabetes medications like insulin or sulfonylureas, which may need dose changes to avoid low blood sugar.
  • Mental health history. Your provider will monitor for mood changes and suicidal thoughts as described in the labeling of some weight-management medications.

Why a full review matters

Your provider will review your history, medications, and family history before prescribing, and will monitor you afterward. Be honest and complete when you fill out your intake.

What if it is not right for me?

Other options may be available, including other medications, nutrition and activity plans, and referrals. At TeleSlimClinicDallas.com, video visits with a board-certified obesity medicine physician team include a safety review. Compounded drugs are not part of our program. See our medication options.
